1. Your complete mailing address
2. Your choice of a boy, girl or no preference
3. Choose a sponsor donation option, i.e. monthly ($30 USD each month), quarterly ($90 USD every 4
months) or full year ($360 USD) one time payment.
4. Number of children you wish to sponsor: ex: one child, two chidren, etc.
